Most forms can be filled out on line and printed.  You may also download the form and save it on your computer. To download click on the link with your right mouse button, then: If you use Netscape click "Save Link Target As"; If you use Microsoft Internet Explorer click "Save Target As".  Make note of what directory you save it to so you can find it latter.  NOTE: Some forms need Acrobat Reader 5.0 or better to fill out. You can download the latest version of Acrobat Reader by clicking here or "the get Acrobat Reader" icon at the bottom of this page. 

One of the most important forms listed is the 7028 Change of Members Information.  This form is used to keep your information such as Name, address and email current in AUXDATA.  Keeping your information up to date insures you are kept up-to-date on the latest news. It is very important! If you have a change in your information such as email address fill out form 7028 "Change of Member Information", print three copies and give two to your Flotilla IS officer for processing and keep one for your records.
